OVERHEAD PRESS - VERTICAL PUSH .

The overhead press is one of the most compound exercises used to train through the vertical plane of motion. It is used in athletic, recreational and rehabilitative environments to train the muscles and structures of the shoulder girdle. A secondary benefit is that the overhead press promotes improved function of the scapulothoracic stabilizers as well thoracic extension. This may lead to improved posture and decreased low back pain (McKean, M.R, Burkett, B.J, 2015)

OMUZ ÇEVRESİ KAS YIRTIKLARI NASIL TEDAVİ EDİLİR?

Omuzda rotator manşet oldukça önemli bir fonksiyona sahip bir anatomik yapıdır. Omuzun çevresini destekleyen omuza farklı haraketler yapma fonksiyonu sağlayan bir kas yapısıdır. Aynı zamanda omuzun stabilitesini , yani humerus başının glenoidle olan ilişkisini de sağlar.
Genel olarak rotator manşet yaralanmalarını iki tipe ayırma mümkün. Bunlardan biri akut rotator manşet yaralanmalarıdır. Dirence karşı omuzun kaldırılma hareketi sırasında olabilir ya da üzerine düşme gibi travmalarla olur. Bu tip travmatik kas yırtıkları genç hastalarda görülür. Diğer bir grup rotator manşet yırtığı da zaman içerisinde kas içinde aşınmalara ve birtakım yıpranmalara bağlı oluşan kas yırtığıdır. Bu hasta grubu genellikle 50 yaş üzerinde hasta grubudur. Hastalar zaman zaman omuz ağrısından şikayet ederler. Hastalar basit bir hareketle örneğin halı çırparken ya da yukarıdan bir şey alırken omuzunda ağrı başladığından bahseder. Bu tip yırtıklar bayanlarda daha sık görülür.
Rotator manşet yırtığında, temel olarak hastalar bize omuzunu kaldıramama gece ağrısı,omuz fonksiyonlarında birtakım handikaplar, hareketlerde birtakım kısıtlıklarla başvururlar. Bu hasta grubunda iyi bir fizik muayene, gereğinde radyolojik değerlendirme ve özellikle MR ile desteklenen yöntemlerle tanıyı koymak mümkün.
Tedavide özellikle rotator manşet yırtığı tam kat değilse yani kasların yarısından azını içeren kısmi bir yırtık varsa, cerrahi dışı tedavileri öncelikli tercih ediyoruz. Bu yöntemler ilaç tedavisi ve fizik tedavidir. Hastalarımızın bir kısmı bu yöntemlerle rahatlayabilmekte. Eğer yırtık tam katı ise ya da, kısmi yırtık olmasına rağmen fizik tedavi ilaç tedavisinden fayda görmemişse bu hasta grubunda cerrahi tedaviye geçilebilir. Cerrahi tedavide son dönemde kapalı yöntemler olarak halk arasında bilinen artroskopik omuz cerrahisi yöntemini kullanıyoruz. Omuzun etrafında açtığımız yarımşar santimlik bir kaç kesiyle, bir takım özel enstrümanlarla kası kemiğe özel (çapa) aparatlarla tuttuyoruz.

💪When we think we have a rotator cuff (RC) injury, we immediately think we should strengthen it.

While that may be true sometimes, do not forget about how your scapular/shoulder blade is moving in tandem with your rotator cuff! ☝️One commonly injured RC is the infraspinatus and subscapularis.

These muscles are responsible for shoulder external rotation.

When these muscles are strong, you will have a strong overhead press or backhand swing.🏋️🏌️ Rehab exercises are only useful when they are done correctly to achieve pain-free movement.

Last Wednesday evening our physiotherapists, Tim and Sean, attended a MPA/APA lecture night on surgical updates of the rotator cuff. 
As part of his role on the MPA committee, Tim convened the evening and spoke on tips for rehabilitation following surgery, as well as complex rehab cases. Thanks Tim! 
A huge thankyou also to the key speakers Anaesthetist, Dr Hsu, and Shoulder Surgeon, Dr Gupta. 
A fantastic evening had by all 🙌🏻

Improvising with my left hand only. 
Another adventure in creativity as my right shoulder recovers.
Just the other day, I posted a video on FaceBook about one of the unexpected gifts of improvising. It’s the ability to adapt your playing with unpredicted limitations.

This incident of a damaged rotator cuff (right shoulder) reminded me of a time long ago when my right arm was out of commission for a whole year. I describe the details in this video.

Anyhow, it inspired me to sit at the piano and try a little improvising with my left hand only. I wish I had been more adept and active with improvising back when I had that other injury. It would have provided so much more musical joy and fulfillment. This is yet another gift of being able to improvise.

Have a listen and see what you think of my left hand only improvisation. I’m only imagining what would happen if I practiced improvising with one hand alone more often. All kinds of new ideas and neural pathways would emerge.

This is an amazing exercise for strengthening the external rotator cuff (#Infraspinatus and #TeresMinor), but when I see people do it in the gym, it is often done incorrectly. Watch the video and read below, then do 10-20 repetitions on each side, starting with your weakest side. Use a light resistance band and increase it over the coming weeks/months.

Keeping the shoulder blade in a scooped position while also keeping the shoulder blades DOWN will put the shoulder in an optimal position for this movement. Many people don’t think about what their shoulders are doing and just simply rotate the arm outward without much thought. This can increase the pinching feeling that people get in this shoulder and you may not isolate the muscles as much.

Make sure that the wrist and arm stay straight so that your rotator cuff is doing all of the work, not your forearm/wrist. Focus on rotating the arm (humerus) and not letting the shoulder blade move. Keep the shoulder blade still. When you rotate the arm outward and don’t let the blade move, you will notice that the range of motion is limited… this is how it should be. You won’t be rotating very far. You will get a gnarly burn on the back side of the shoulder. .
