El tejido cartilaginoso que incluye tanto al que cubre la superfície articular como los meniscos, es un tejido que posee una alto contenido de agua, eso le confiere al material la posibilidad de servir como amortiguador, debido a que absorbe energía durante los impactos.

Como se aprecia de ese delicado equilibrio entre composición, forma y función, cualquier alteración a los mismos, aumenta la posibilidad de lesión en un tejido.

Factores como la edad y el sedentarismo, modifican la relación colágeno-elastina,  lo que reduce las presencia de agua en los tejidos volviéndolo más rígido y con ello más propenso a una lesión.ㅤㅤㅤ
La presencia de daños previos (lesiones anteriores), así como cambios en la estructura del material (ej. osteoporosis), también modifican las propiedades mecánicas del mismo, haciéndolo más frágil en este caso.

Training with a sore or injured back is tricky. If you were to avoid working it altogether, your workout would likely be comprised of vacuums, dead bugs, and pulldowns. There are plenty of modifications you can make to existing exercises that'll keep back issues at bay while still pursuing good numbers in the gym. Here are 2 we recommend.
‼️‼️‼️‼️These tips aren't for people who are still immobile due to throbbing pain. These are for people who are on the mend and are still able to do stuff and have been diagnosed by a physician/physiotherapist/clinician to continue exercise‼️‼️‼️‼️ ✳️Decompress Often
After finishing a low-back heavy, compressive exercise, the first thing you should do is just hang somewhere, like on a pull-up bar. It's a natural response in wanting to feel better, take pressure off the spine, and stretch the surrounding muscles at the same time.
Be mindful of how much compression you put your spine through each workout. The cumulative amount can come back to haunt you, even if your back is healthy.
Exercises that create upward forces on the spine instead of downward forces should be included in your workout. Here are some good examples of movements that can still provide an appreciable load for hypertrophy or strength gains: •	Parallel Bar Dip •	Pull-Up or Chin-Up •	Lat Pulldown
✳️Ditch the Lifting Shoes
The lift provided by lifting shoes can create a more vertical torso and a more quad-dominant and less low-back-dominant squat. That sounds like a good thing, but it can cause a lifter to rely less on his own mobility to reach full depth in his squat.
Anyone can squat deeper with what appears to be good form when his heels are elevated. Chances are, especially if your low back is aggravated, the ass-to-grass depth may be a bit of a stretch for the muscles, tendons, and ligaments of the lumbar and sacral region and could lead to further issues.
It's better to keep the heels flat on the ground, work on improving true mobility, and limit your depth to just below parallel until you've made those improvements.

What could be the cause of heel pain in kickboxers? 
Posterior ankle impingement is pain localised to the back of the ankle. It is often due to either bone or soft tissue being compressed during plantar flexion (i.e. foot pushing down), a common movement pattern seen in kickboxing. SCAPULAR WALL SLIDES W/ LIFTOFF:

This is a great scap stabilizer and cuff activation exercise when you're ready to transition overhead or you want to prime the stabilizers prior to any horizontal and especially vertical pressing.

The big plus with this movement is that activating the arms against the band instantly improves scapulothoracic movement (shoulder blade on the thoracic wall) and will eliminate any winging if there is some presentation (unless there is nerve involvement).

I generally start my patients with just the wall slide vertically and when proficient, have them then perform the lift-off portion at the top. Generally the lift-off is performed in a slight Y pattern to create an angle that will facilitate more lower trap activation.

FALL AND BALANCE PROGRAM
Our Falls and Balance Program runs over a 6 week cycle, with patients able to commence the program at any time.

To begin the program, participants attend a 60 minute initial assessment with our Physiotherapist to determine the contributing factors for the participants increased falls risk. Following their initial assessment the patients can commence the Falls Prevention exercise and education program.

At the end of their 6 week program the patient will have a 30min session with the physiotherapist for reassessment, to determine improvements made and any ongoing areas of weakness or concern. Classes can be continued after the initial 6 weeks if patient or Physiotherapist deems appropriate.

Guess who showed up today ready to go with a new prosthesis 👧🏻!!! With this being her 4th leg, she’s done much better getting comfy. But she was reverting back to her old gait pattern and not going through a complete stance phase to allow the knee joint to flex. So to help get her over the hump, I decided gave her a little sensory cue.

While the distal part of her leg 🦵🏽 is physically gone, its representation in the sensory cortex 🧠 still exists. So I decided to use tape on her limb to bring some awareness to that region. She still has some hypersensitivity in her stump so I chose the jellyfish taping to cover area but not overwhelm her. By the end of the session she was taking cues for her gait pattern and even started jumping!

🥎 ACL REHAB 🥎

We talk about a lot of in depth details when it comes to rehabbing after an #ACL tear. While a lot of the intricate and sport specific type approaches are important, the one thing that MUST happen during all ACL rehab is the development of strength
.
We chronically under load patients in traditional rehab. We’ve all been guilty of it and it’s something I’ve tried to really focus on with all my athletes as they work through the rehab process and get back to sport
.
Here, I’m working with @tayokada nearly 3 months post op on a combination of double leg and single leg strengthening
.
1️⃣ Rear Foot Elevated Split Squat: we took a tempo approach, 5 seconds going down, 2 second bottom pause, and 3 seconds coming back up
2️⃣ SL Eccentric Hamstring Sliders: focus is on getting that knee to go from bend to straight with as much control as possible, not worrying (yet) on returning to the starting position
3️⃣ Double Leg Hip Thrust: I’m a firm believer that this should be a staple movement for ALL athletes and humans
4️⃣ Barbell 📦 Squat: this is a good way to load the tissue while being mindful of depth. Put the box where the pain typically would begin if you have a hard time controlling depth

Ready for the answer from yesterday’s FIND THAT LAMENESS 🧐??

Here goes: the short answer is RIGHT HIND!

So how did I know that? Lucky for me I had more information than you all. I can see the scrape on his right hip from a recent injury, I can see that at rest this horse chooses to put all his weight on his left hind and cocks his right hind. I also watched him trot, where he favors his right hind significantly more.

I know, that’s totally unfair! BUT there is a way to tell from the walk! I think a lot of folks guessing left hind got a little confused because this issue is not in the hoof, it’s in the stifle and possibly hip.

As I watch him walk, first I’m looking for how far forward he’s stepping under. Looks about the same on both sides.

Then I look at how long he’s keeping the leg on the ground in the extension phase. The angles in the photos show how he’s not getting full extension of the right hind limb as he walks.

This was a tricky one, guys! Congrats to everyone who got it right!!

🔰Functional wrist extension ROM requires approximately 70 degrees but in some tasks requires more. The culprit can be muscle length deficits of the wrist flexor group or accessory hypomobility of the wrist joint itself.

🔰A quick wrist mobility test to check for sufficiency or comparing side to side is demonstrated in video 1 with the “prayer” assessment. Let's think about that for a second 🤔 yeah-no. Unless they are sucking it out, not gonna happen. People are soooooo desperate to "lose weight" but soooooo many are going about it the wrong way, or are not willing to do the things they need to do to LOSE FAT...and yes, there is a difference. Weight does not equal fat.
.
If you weigh yourself one morning and the scale spikes up (even after a "perfect" day of eating), it does not mean you gained fat.
.
Your weight will constantly go up and down and left and right for no other reason than you're putting things in and squeezing them out of your body on a daily basis. 🤯 # science.
.
Truth is, losing weight is easy. You can get the scale to say you weigh less by sitting in a hot tub and sweating for 15min. 🤷‍♂️ You can lose weight by cutting salt and carbs from your diet so your body holds less water. You can lose weight by getting food poisoning. You lose weight every time you pee/poop, and even on those special occasions when you do both at the same time. 🤣

Focusing on your weight is a surefire way to set yourself up for failure and disappointment. Not allowing yourself to be happy until you reach your "goal weight" will mess with your head and emotions and do more harm than good.

Use the scale as a tool, but you need to remember the scale means nothing compared to how you look and feel. The scale is not always indicative of fat loss (or gain). If you only focus on trying to move the scale, you'll end up looking for short-term fixes instead of long-term solutions.

We can gain or lose fat and we can gain or lose muscle, that's it. 
If your goal is to lose fat, focus less on moving the scale and more on hitting your calories and protein. Strength train 2-4x/week (and this doesnt mean the fufu HITT with no progressive resistance). Be flexible with your diet, progress comes from consistency, not rigidity. The sooner you understand this is a life-long process (not a 7-day challenge), the sooner you will be leaner, stronger, healthier, and happier with your body. You didnt get fat from 1 cookie, so eat one 🤷‍♂️

ACL rehab - get stronger

Once the foundation of full range, solid quad contraction and reduced swelling is set, progressive strengthening must occur. In my opinion, this is one major area lacking in ACL rehab. Many athletes are under loaded which causes them to fall behind in their rehab. Getting stronger should always remain a key as strength can significantly lower the risk of re-injury.

Progressive loading and stress management are two huge keys to rehab. I want to make sure each session is challenging but does not irritate the knee. The quads and hamstrings are the two most commonly effected muscle groups and require the most work to regain full strength. Getting feedback from patients is important as we monitor progress. Here are three exercises I like to use in the mid phases of rehab that hit the quads, hamstrings and glutes.
1️⃣ Banded single leg RDL - this is a higher level RDL. The band increases hip demand for single leg stability and the two kettlebells loads the hamstrings. 
2️⃣ Eccentric reverse step downs - working on single leg control as she steps backwards. I cued her to explode up once her foot touches the ground.
3️⃣ Hip thrusts - commonly used glute strengthening exercise. The band around the knees increases glute demand.

Hello! Meet David Nolan @big_roast24, from UK who is travelling around Asia for almost 6 months now. He was in KL for a week and decided to drop by our centre before he embarks on his tour again! .

He has been a competitive rugby player for almost all his life, since 20 years ago, and has been to physiotherapy treatments ever since! He came with the complain of tight hip flexor and a pinch sensation when he walk that annoys him. We did dry needling and sports massage for him. .

It was a pleasure to cross path with him! He was here with us for 3 sessions, and we mainly focused on education so that he can bring the essential messages with him whenever he travel to further sustain the release that we have done for him!

With the PTCAS application system opening up in about a week, I know many of you are currently in this situation seen above. ⁣

Throwback to about a year ago, I myself was overwhelmed with excitement and fear all bundled in one. At this time I was finishing up my last prerequisites for school, studying for my GREs, completing an internship,and working full time in an ortho clinic. All the while I was scrambling to get ready to apply to a multitude of DPT programs. ⁣
▪️⁣
During my undergrad I was the president of my universities Pre-PT club. I often had people ask me for advice about application processes. Here are some simple tips I frequently provided. ⁣
▪️⁣
1️⃣ Apply as EARLY as possible. Most schools are rolling admissions even though it’s not always clearly outlined. ⁣
2️⃣ Have strong LORs, these go such a long way as they give schools an insight to your character. ⁣
3️⃣ Have variance in your observation hours. Examples are : outpatient , inpatient, neuro, pediatrics, etc⁣
4️⃣ Get your essays reviewed by multiple people. A strong essay can be a game changer. ⁣
5️⃣ Be persistent and resilient. Things will get tedious and strenuous but in times like those you need to continue to keep your head down and work harder than you ever imagined. You can always accomplish more than you can fathom. ⁣

EDUCATING YOUR PATIENTS ABOUT PAIN 
I have so many patients with arthritis and that is all I hear…. “I can’t do this. I have arthritis.”
“I’ll never get better because it’s arthritis.”
I can’t walk because I have arthritis.” Where do they get this information! 
It is our job to educate our patients on pain! 
Use the time during manual therapy to educate your patients on pain. 
Your patients are taking a rest during exercises? Educate them about pain. 
For most patients the more they know the better your outcomes will be. 
Let them know the pain cycle. Let them know things you think may not interest them. Because it probably does. 
Most of our patients have had this for a while and the more you get them to feel in control about handling it the better end result you will get. 
And again this makes it skilled!!! Education during rest breaks is billable! 
So do it! 
Document it! 
Get your patients better results because of it!

[Kinesiotape for Ankle Swelling- Squid Technique]

Kinesio Taping is one of the best ways to manage swelling at the ankle joint after an ankle sprain or after surgery!! Patients and athletes call it “the magical tape” for a reason. Although manual techniques are helpful, kinesiotape is an excellent tool to improve lymphatic drainage and get rid of swelling quickly. 
Kinesiotape can be applied in many ways, but in this particular technique we are demonstrate “the squids” using barely any tension on the tape, which is used to help pull the swelling up and away from the joint you are working on.
