Lactate has been presumptuously villainized by the athletic community, inaccurately implicating “lactic acid” (what lactate converts to when taken out of the body for analysis) in soreness and tissue break down. It’s a destructive and ill informed opinion on a truly remarkable compound.
•
Your body produces lactate during anaerobic glycolysis; energy production in the absence of oxygen. Lactate is then shuttled to the liver and converted back into glucose, once again becoming a usable form of energy. In addition, preliminary evidence has shown lactate’s utility in stress relief, fighting depression and most recently, in brain derived neurotrophic factor (BDNF)! Lactate was able to make its way into the brain and mediate the release of BDNF in the hippocampus of rats (part of the brain that controls memory).

If you’ve followed my recent posts on intense aerobic exercise and its benefits for neurologic disease, BDNF seems to play a major role in neuroplasticity and task learning. Up until the release of this article, the mechanism for BDNF release in the brain was largely unknown. Although not perfect, these animal and laboratory models may provide catalyst for future investigations into the role of HIIT and recovery from neurologic disease in humans. More steps need to be taken, but this is a promising lead into how we can best help our patients!

Continuing with the common theme of the past few weeks, I present more evidence that endurance training, specifically on a bicycle, is capable of improving people’s lives with neurological disease.
•
Dementia, Alzheimer’s Disease being the most common subtype at 60%80%, is loosely defined by general cognitive decline. Incidence increases with age and causes are dependent on the subtype. Dementia is a frustrating condition for both the victim as well as those responsible for their care. Memory, independence, personality and the ability to communicate become lost as the disease progresses. Majority of dementia subtypes have no cure. Treatment comes down to limiting progression and secondary complications.
•
As we’ve talked about previously, aerobic exercise is a great way to induce neuroplasticity. There have been many proposed mechanisms that exercise improves outcomes in people with dementia. Regardless it is fairly conclusive that exercise is safe and effective in this population. The article outlined above shows how a long term program of just 15 min of light cycling (stationary recumbent) every day (88% attendance) can significantly improve or mitigate loss of various important functions. This article simply demonstrates another win for aerobic exercise within neurologically impaired subjects! •
The greatest difficulty with people with dementia is getting them to adhere to an exercise program when cognition is largely impaired. This was evident in the large drop out rate (30-50%). Intention to treat was performed and significance was still found in the same measures.

Man did I want to externally rotate! But I wanted to mimic pedal position as best I could, so tried to keep the feet pointed forward.

Don’t be afraid of lifting heavier weight. To get any strength benefit you have to be lifting >60% of your one rep max. Preferably closer to 80% 🤨. Misconceptions of strength training... Weight gain 👎 
Get injured 👎 
Lose fitness 👎
Elite cyclists don’t lift 👎 
Just do it!!!!!!!!

= Stabilità di appoggio sul manubrio e, di conseguenza, di tutta la parte superiore del tronco. ➖energia si spreca per stabilizzare il busto, ➕ne rimane disponibile per spingere sui pedali.

Questo si ottiene in primis con una corretta scelta dell’attacco manubrio e degli spessori da utilizzare e, in seguito, con un’adeguata scelta del manubrio assieme ad una corretta regolazione delle leve, che deve variare secondo il brand scelto.

There is so much to be gained from understanding what optimal conditions are and when we can take advantage of them. Think about how you plan your meals to optimize a training session, ie eat a large meal ‪2-4hrs‬ before training, get some carbs in during a long/hard workout and then refuel appropriately afterwards ⛽️. Creating an environment where your body can succeed and timing it appropriately is a difficult but crucial determinant of growth 🌱.
•
Last week I mentioned “neurotrophic factors” related to parkinson’s disease. This post is going to elaborate on what these factors are and how we (healthy and diseased) can use them to our advantage. The most talked about neurotrophic factor is the “brain derived neurotrophic factor” or BDNF 🧠. The slides above describe what it does and how best to up-regulate BDNF.
•
Key things to note on the second slide are the dose responses and the timing. Higher intensity exercise (as a percentage of VO2 max) creates a greater increase in BDNF post exercise than low intensity. That being said, diseased/disabled populations such as subjects with MS and spinal cord injuries, may still benefit from low-moderate intensity exercise. Secondly, these increases in available BDNF only stick around for 15-60min POST exercise! This means you have to act quick. The third slide is a graphic from the article last week that helps to put everything together.
•
There is a period of time where the brain is primed to learn and grow. This has its implications in how healthy people store memories/learn as well as diseased/disabled people relearn simple tasks such as walking or using a spoon. 15-60 min to take advantage! So maybe next time you are trying to learn something new, like playing the piano, do it right after a hard workout when your brain is overflowing with BDNF, or learning juices 🥤 as I like to call them 🙊

As a cyclist I’m not about to enter myself into an arm wrestling match 💪 🤼‍♂️, but these wires still have to be strong! Our hands are almost always in contact with the bike, making them an important body-bicycle interface.

In my opinion, spending time bearing weight through your arms is a great way to build functional capacity in your core as well as your upper extremities. The exercises above are easy ways to incorporate this into your strength routine. 1️⃣ planks 2️⃣ mountain climbers ⛰ 3️⃣ push ups 😭.

In addition, we know that muscle contractions can improve bone mineral density. Take push ups for example. The pecs have a large attachment to the clavicle (collar bone 🦴 ), a bone that many cyclists are well acquainted with 😬.

Don’t skip arm day! Here are a couple exercises that spend a lot of time with weight through your arms.

Who remembers creating your own freecoaster? I used to break these down and remove the coaster brake and make it a "freecoaster". Freewheels were better but sometimes failed. So plan B was this.

I was at the @aptapics annual combined sections meeting (CSM) in DC this past weekend. There was so much cool evidence being presented on aerobic training and its benefits for people with neurological impairments (stroke, traumatic brain injury, Alzheimer’s, etc…). I thought I’d share something I learned about cycling 🚲 and Parkinson’s Disease (PD).
•
PD occurs when the part of the brain 🧠 that produces dopamine begins to deteriorate (dorsolateral striatum). This results in signs such as tremors, slow movements, and freezing while negotiating things like curbs, doorways and changes in surface. It is a progressive disease with major consequences in later stages.
•
The idea of forced exercise for PD started with an animal study. They chemically induced PD in rats 🐀 , got their baseline work rate on a treadmill and then forced them to exercise at 130% of that. If the rats slowed down, they would be given an electric shock ⚡️ . They found remarkable improvements in the rats, namely due to up-regulated “neurotrophic factors” in the brain that are critical to brain health/growth 🥬 . Up to that point, aerobic exercise had shown limited benefits in humans with PD.  But those studies were all “voluntary”, not forced. But researchers were not about to shock people with PD for the sake of science 😬.

That was until a neuroscientist and person with PD biked across Iowa on a tandem bike. The person with PD went on to comment that at the end of each day she felt like she didn’t have the disease for a little bit. Turns out the key was she was pedaling at a cadence ~25rpm higher than voluntary, as she was forced to pedal at the same rate as her tandem mate in front of her. This prompted the study outlined above. In one sentence: high cadence is more important than HR or power in improving impairments related to PD.

Band side stepping progression
•
Bands are a great tool, especially when you don’t have access to a lot of equipment. These are great for your hip abductors (glutes 🍑)! Power in cycling comes from muscles performing extension/flexion (sagital plane). That’s not to say other movers can be neglected 🤔
•
Progression of the exercise comes with band placement. As the band finds itself further away from the axis of movement, the longer the lever arm, the harder you have to work against an external force 💪 
1️⃣ around the knees
2️⃣ around the ankles
3️⃣ around the feet 🦶. This arguably focuses even more on the muscles that rotate your hip out.

These are muscles that need to be non-fatiguing so high reps (15+) low resistance for these! Gotta feel the burn by the end though 🔥. See some earlier videos on exercises that work the gluteus medius best! Yes, my socks aren’t matching 🧦, and yes I just got off the trainer 💦 •

Split squat progression.
•
The split squat is a great exercise traditionally highlighting the quads 🦵. Any time you shift your weight onto one limb you demand your lateral stabilizers (glute med/min and lateral trunk flexors). I love single leg exercises for that!
•
Progression:

1️⃣ isometric (the joint isn’t moving). Great for building early strength in rehab but is limited by the angle the joint is working at.
2️⃣ isotonic (joint is moving with roughly similar muscular input both up and down). Great for building strength 💪. Add weight to make this harder 🏋️‍♀️ 3️⃣ explosive (great for power = work/time). Speed is 🔑 •
I’m going to try and do an exercise progression each week. It’ll be a nice challenge for consistency on my part. I’ll do most of it with minimal equipment but might sprinkle in some stuff from the gym as well. Also just got off the trainer, hence the bibs 😬

Se você é iniciante no Pedal, calma! Não queira já sair da loja “clipado” em sua nova bike. Você pode ter muitos benefícios do seu pedal Flat (plano). Um estudo de cientistas coreanos publicado no Korean Journal of Sport Biomechanics (2016), avaliou a amplitude de movimento das articulações e a ativação muscular em ciclistas iniciantes usando pedal FLAT (Plano), Clip (Pedaleira) e CLEAT (Pedal Clip popularmente conhecido). As avaliações foram feitas em nove ciclistas pedalando 3min com mesma carga e cadência de 60 rpm. Foram avaliados a ativação do Vasto Lateral, Tibial Anterior, Biceps Femoral, e Gastrocnemio por eletromiografia durante quatro fases da pedalada a partir dos graus de giro do pedivela.

Resultados mostraram maior amplitude de movimento do joelho e ângulo de dorsiflexão máximo do tornozelo pra quem usa Pedal FLAT comparado aos outros dois tipos, isso quer dizer que o ângulo de Flexão do Joelho será menor, diminuindo a pressão na capsula articular. Já o tempo de Flexão Plantar foi maior para Flat e Clip do que para Pedal Cleat, por conta de uma menor ativação do Tibial Anterior, que para ciclistas experientes o tempo de transição é mais rápido para dorsiflexão, o que mostra maior eficiência dos pedais Cleat.

Já a ativação muscular especialmente dos posteriores de coxa, não houve diferença em iniciantes em nenhum tipo de pedal, isso se explica pela maior ativação do quadríceps e especialmente do Vasto Lateral para força de empurrar o pedal como força principal de deslocamento em inciantes. Diferente dos atletas de elite, que usam a técnica com mais eficiência na distribuição de carga entre as 4 fases da pedalada. 
Apesar das limitações do estudo em usar bike estacionária, nos mostra que é necessário que iniciantes usem os pedais flat com estratégias de aprendizado para otimizar a técnica de pedalada, para quando chegar o tempo de pedalar com Cleat Pedals, obter máxima eficiência proporcionada por estes pedais, assim como pedalar com segurança sem riscos de quedas.

Book club: “all in your head” by Suzanne O’Sullivan, a $2 find on some clearance sale in NYC. It’s a fascinating series of patient cases outlining illnesses that have no biological/physical cause... ie all possible tests come back negative, meaning there’s no “medical” explanation for an illness.
•
We all experience, to a minor extent, what’s considered “somatosizing”. Heart ache after an emotional loss, for example, may indeed manifest as chest pain despite nothing being wrong with your heart. Blood may rush to your cheeks when embarrassed. These are examples of benign physical manifestations of emotional states. For a select few, however, somatosizing can lead to severe illnesses such as seizures, blindness and chronic pain (all were cases in the book). These are diagnosed as psychosomatic disorders. They are often the physical manifestations of suppressed emotional or physical trauma, a protective mechanism. Effective treatment comes in the form of graded exercise and or cognitive behavioral therapy.

This isn’t something a physical therapist will treat independently nor something an athlete will encounter regularly, but it provides light to the power of the mind. Despite nothing physically going on, people have been debilitated. The opposite end would be the body’s ability to suppress illness despite incredible physical trauma. Think of Phillip Gilbert riding 60km with a fractured knee cap! Your mind is ultimately in control.

Of note, the book also brings to light the barriers of treating psychological disorders, namely societal stigmas discouraging patients from accepting psychosomatic diagnoses. It’s a theme that’s plagued mental illness on all fronts, and something I’ve seen in my personal clinical experience. Even if there is no physical explanation for an illness, it is still very real. It’s in our best interest, despite our potential hypotheses of malingering (disability, workers comp), to treat those patients equal to all others. And for those who aren’t clinicians, it’s important to accept that those who suffer from mental illness, truly suffer an illness just as one would suffer knee pain or have diabetes. They deserve our care and attention.

@bikecad is great for designing bicycles from the tubes up. That being said it is difficult to input an existing frameset without manufacturer dimensions (especially if you don't have a fork). To get around this we wrote a Mathematica script that gives bb drop, head tube angle, seat tube angle, front center distance, fork length and rake from simple measurements that can be made with a tape measure.  When benchmarked on a Kona Jake the Snake the math agreed with manufacturer angles to within .2 degrees and lengths to within a few millimeters!

Activity dosing is the #1 strategy for injury prevention. Too little stress and you won’t adapt. Too much and you risk tissue failure. With 3wks of traveling, I’ve resorted to running in the hopes of maintaining some form. Shoes fit better in a suitcase!

As an overzealous masochist, I knew going straight from cycling to running wasn’t an option. Cardio carries over, but running has ground reaction forces and eccentric contractions, loads cycling doesn’t provide. I therefore needed a transition period.
•
Nov. 24th (DD) to Dec. 13 (train to NYC) my training goals were to 1️⃣ have a solid cycling block to carry fitness through time off the bike 2️⃣ prepare for lots of running.
•
On top of 8-13hrs of riding/wk I threw in some jogs and strength work. I would jog commute to the bus stop on winter mornings and do 10min on the treadmill before my biweekly gym days. On top of that, I sprinkled in a couple of 4-6 mile runs in those weeks. The gym workouts transitioned from focusing on speed pre-event (DD) to tempo post event, allowing ample opportunity for eccentric adaption.

4 days in NYC
Day 1️⃣ 6.7 miles @8min pace
Day 2️⃣ 5K PR @ about 6:40pace
Day 3️⃣ no running, 40 min weights
Day 4️⃣ 10 mile run @9mim pace: most I have ever done by far (fly to Beijing that night)

After a 14hr flight I feel stiff... but who wouldn’t? My Achilles is slightly swollen on the right but no discomfort. Did I overdo it, or did I give enough time? Thoughts?

Working on a lecture for University of Pittsburgh Athletic Training students. Here are a few slides on nutrition out of my talk on bone health in endurance athletes.
•
Calcium is a tightly regulated element due to its importance in processes such as nerve conduction and muscle contraction. Any drop will cause “wrecking crews” to take calcium out of vast bone stores and redistribute them in circulation. This is a critical function, but relies on diet to replenish these stores. Without appropriate replenishment, chronic losses in bone calcium results in compromised strength.
•
Cyclists can lose 69mg of Ca per hour through sweat at 70% ventilatory threshold. Diet is the only way we can replenish calcium. Calcium supplementation alone, however, won’t fix the problem. We need vitamin D to absorb calcium in the intestines. All in all, however, diets higher in calcium haven’t correlated with improved bone density in cyclists.
•
Low energy availability has been shown to be detrimental for bone density, making it important to address widespread disordered eating in the sport. All of this just “food” for thought

My longest Double Century to date🤗 and yes I had tons of fun!

It all stared out as an idea that I shared with my clients almost a year ago. At the time all of them looked at me like: “Right, clearly this pregnancy thing is really getting to her”😅 Entry in hand every person committed to putting in the work. Some doubting that two indoor classes and some riding over the weekend would simply not be enough!

Yes, every single person kept on asking, “do you really think I would be able to do it?” My answer always the same... “I wouldn’t have asked you if I don’t think you can do it, I really believe it is possible.” Our full team finished in a time of 8h51m🤗 as a coach and mentor... I am happy with that 💥

Let’s talk about bones 💀 again! I’ve mentioned that due to multiple factors, high volume cycling can lead to low bone mineral density (BMD) under the “right” conditions. It has been proposed that ground reaction forces (GRFs) and muscle contractions lead to osteogenesis. New evidence hypothesizes that torsional forces could be the key to bone growth!
•
Cross sectional studies have demonstrated improved BMD in tennis players and baseball pitcher’s dominant arms compared to the non-dominant, specifically the humerus. Both activities demand large torsional stresses as opposed to GRFs.
•
Shift to the lower extremity. One study has shown that the greatest torsional stresses at the tibia occur during forefoot sprinting (running), going up stairs without contact from your heal, and isometric calf contractions. This indicates that certain muscle groups could be predominantly responsible for torsional stress at certain bones.
•
Shift to the video above. Put some of those conditions together (stairs without heal + demand on the calf muscle + high power = single leg stair hops). Under the assumption that torsional stresses are key to bone growth, my tibias are making GAINS! Not to mention the power benefit 😏.
•
Can I point you to RCTs that show stair jumps result in better BMD in cyclists? No. But the best we can do is put puzzle pieces together until something better comes along. •

I’m not one to get super stressed, but an exam today had me on edge the last few days. I, however, didn’t change my workout schedule this week. Yesterday = revitalizing hour of power on the bike after a long day of studying 📚. Today’s weight training post exam, however, was a struggle 😴.

First, how are they a good pair 👰? A stressor is anything that moves your body away from homeostasis. Our response to stress is to activate our fight 🤛 or flight ✈️ system. In order to do either, we need energy readily available in our blood, so we mobilize energy stores from our liver, muscles and body fat. If a lion was chasing us, we would appreciate the available energy as we sprint to safety 🏃, but stress in modern society is different. Modern stress is anticipation 😟(exam anxiety). While I was worrying about my exam, my body was mobilizing the troops… but none of it was getting used because I didn’t need to “fight or flight”… My body doesn’t know the difference! If you allow these unused morsels of energy to circulate, this can lead to chronic diseases such as type II diabetes and atherosclerosis ❤️ 💥. But if we exercise, we can use up the energy built up from stress and help prevent future issues.

But why couldn’t I perform in the gym when I usually don’t have issues? Exercising, like stress, is a non-restful state 😳 (no duh). But if stress takes over off the bike, the body has no time to turn off. Energy stores can’t be re-filled and our central nervous system fatigue 😴. Several days of stressful studying, an hour of power, an exam and no “off time” in between and I had nothing left for the weights. Stress + rest = success… and with that, I go to bed. I’m tired 🛌 🌚 •

Regardless of how stable you feel on a bicycle, your body is constantly having to make micro adjustments muscularly depending on how variable the terrain is. Think MTB vs smooth road. The trick is knowing how to produce power in these conditions. Accessory work should always address this inherent instability. Here is a sample of a few of the exercises I included in my workout today.
•
1️⃣ squat on a bosu ball
2️⃣ Romanian dead lift
3️⃣ box jump with a static start (power more than stability)
4️⃣ quadruped dumbbell row
•
Strength training has so many benefits for a cyclist... think improved power, injury resistance, athletic variance, improved bone mineral density etc... no excuses

Fresh outta the CNC! Prototype adjustable lower head tube to test steering ratio curves.

Dealing with so many new variables is why past linkage designs have encountered challenges. We're doing our homework to ensure this is the first front linkage to be free of quirks and to realize the potential of the concept.
